Certificate of Fitness Online Appointment Scheduler

You must complete a Certificate of Fitness (COF) application and pay online for the exam before scheduling an appointment. Without showing an online COF application ID, you cannot take the exam, and you will have to reschedule.

For all appointments, an FDNY COF application must be completed and paid online. Once you complete the application, you will get an FDNY COF application ID with your confirmation email.

• The FDNY COF application ID is shown on the email title, the format is "2xxx-COF-xxxxxxxx-APP".
• The FDNY COF application ID must be entered in the CONTACT DETAIL of the appointment scheduler.

Appointments are between 1:10 pm -2:00 pm, five days a week (M-F) except for legal holidays.

Appointments are required for the following COF exams. You must upload the required documents listed while you are applying for the COF. If you are missing any of the required documents, you will not be allowed to take the COF exam.

N-85 (The first part of the FLSD computer-based exam)

• The latest 31-hour FLSD course school certification/letter

Z-89 (The second part of the FLSD computer-based exam)

• The FLSD application form
• The FLSD verification letter OR you can upload a copy of the following card(s): F58/F25/F59/Q85/F85/Q89/F89/T89 instead of a verification letter
• This N-85 passing letter
• The latest 31-hour FLSD course school certification/letter

F-80

• The latest F80 course school certification/letter
• The F80 application form
• The F80 verification letter

S-56

• The latest CSFSM course school certification/letter
• The S-56 application form
• The S-56 recommendation letter
• The required document(s) listed in the S-56 application form to verify your experience.

Z-51 (The first part of the RSOE exam)

• A high school diploma or GED
• Universal Technician Certification
• A completed Z-51 application Form
• One of the following documents
  • The RSOE course school certification.
  • Copy of the required license(s) listed in the Z-51 application form.
  • The recommendation letter specifying the required experience listed in the Z-51 application form.

  S-98

  • A recommendation letter on an official letterhead that is issued by an FDNY approved Fire Alarm company. The recommendation letter must indicate that the applicant has at least 3 years of full-time experience in the programming, testing or servicing of fire alarm systems.

  Although it is not required, appointments are available for all other COF exams.

  When you visit the FDNY Headquarters

  • The government-issued photo ID (e.g., a State-issued Driver’s License or Non-Driver’s License or a passport or NYC ID).
  • The appointment confirmation with an FDNY COF application ID (print-out or shown on your cell phone).
  • The original copies of the uploaded documents.
  • The required documents listed on the Notice of Exam and the Study Material.
